Cycle Report 09.doc - to be completed each month; one for each cycle in progress.
Materials Request.doc - to be completed and submitted at least three weeks prior to start of a new cycle.
Survey Record template.xls - to be completed for each All Star program cycle. Submit with Pre-test surveys and update / resubmit with post-test surveys.
Sample Permission Form.DOC - can be used with a site registration form to obtain parental permission for child's participation.
